Where should I record Calibrate’s commitment dates and clinical follow-up separately?

Calibrate's initial individual program commitment and a patient's clinical follow-up are different timelines. The commercial offer is $199 per month for the first three months, totaling $597 in installments. The care team must explain the review schedule appropriate to the actual treatment.

Keep the contract dates visible

After the initial commitment, the pricing page describes monthly renewal. Save the start date, remaining obligation and procedure for stopping future membership. A conditional money-back offer needs its own terms and should not be treated as an unconditional refund or medical guarantee.

Prescription medicine and laboratory work are outside the membership fee. Insurance navigation can assist with the process, but the actual benefit decision and pharmacy price remain separate pieces of information.

Keep the professional roles visible too

Calibrate advertises individual video coaching, a curriculum and medical support. Ask which contact handles a practical habit question and which handles a clinical concern. Record how a question moves between those teams when both need to be involved.

The visit-note guide helps distinguish agreed instructions from open tasks. A coaching appointment confirmation is not the same as a prescription or pharmacy dispatch notice.
